First, this isn't a specific Twitch.tv app. This is the same Justin.tv app that's been available for awhile.
Second, they charge $4.99, then show ads to you anyway.
Either make a free app that shows ads, or remove the ads from the pay app. Can't have it both ways, Justin.tv ....
