If you're strumming on the touch screen while it's vertical, you pivot the top screen so you can look down and see the notes and leave the strum bar in the appropriate place. Although this means they'd need to fit everything on the top screen. But this would make sense because you'd be turning the speakers on the DS towards yourself, instead of everyone else hearing it.

I'd play this at home, but I'd bring it to school and totally show of all day.

I picture a small dock to place your DS (probably lite only unless they have an adapter) that can rotate for right or left handers and then have the keys on the left of the DS and the strum bar on the right so your hand doesn't block the screen. The dock could possibly even be mounted be placed at an angle so you can take advantage of both screens. Star Power and such on the bottom, notes on the top. This might have you constantly looking down at your mini-guitar but I think it's possible. Not as good as the console version but then again... are handheld versions ever as good?

I remember here a while back, Kotaku posted an artist's concept of what it could look like, with the neck sticking out one side of the ds, and the strum bar out the other. When I play guitar hero I tend to jerk the guitar fairly hard when I double-strum. I know many others that do as well.

My guess would be something that you sit on a table (as SaintWaldo said) and hook up a peripheral too... Who knows?
