Karakter is a German studio who have been working with HBO on Game of Thrones since the show's inception. They've just released their work on season 4, and it's fantastic.

It's interesting hearing some of the commentary that goes along with these images. Moat Cailin, for example, was shot in a location that didn't match the description given in the books.

"The set we had available for Moat Cailin did not match the description in the books", says Tobias Mannewitz, a VFX concept artist at Karakter, "but it gave us the opportunity to design the castle after a Motte and Bailey model, which is not just a nice phonetic match, but also a castle type that fits the world perfectly."

You can see more of Karakter's art at their studio site. Note that while this art is all Karakter's work, not all the designs necessarily are; a lot of the show's big locations, like King's Landing (pictured below) were done by Gemma Jackson and her team.
