A step counter that has a USB connector on it - plug it into your computer and every step would be an experience point - and you would have a character that you could level up.
This could be implemented into MMORPGs to promote people getting away from the computer.
Considering how crazy people get over "leveling up" we'd have a bunch of thin people in no time, right? Now...how to implement loot.
Anyway, I'm so glad I registered just to post this.

They had this at the Playdium in Missisauga a few years back, dunno if its still there. They hooked up a Hologram circle to Tekken 3.
Basically, you stood in this circle, and if you threw a punch, it made your guy on the screen throw a punch. If you ducked, you ducked. If you kicked, you kicked. Take a step forward to make your character walk forward, step back to step back.
Was kind of neat actually. Made my body hurt from flailing about afterwards.

In fact, if I wishlisted anything, it'd be for "running controllers," (two handwraps with buttons) so that I could actually jog as opposed to walk.

as i've grown older, i believe my game consumption has actually increased(!) while my circulation has grown poorer. a while ago during a Puzzle Pirates binge, my right ankle swelled considerably and i ended up with a cyst on top of my foot. today i sometimes look at my slothfulness while playing hours of Fallout or L4D and i think, "this is really bad."
i know i wouldn't mind a little more exercise, as long as the device is accurate. for that matter, it'd be cool if you could crouch and your character would follow suit (if the game allows for it of course). it'd suck for going prone, though.

Somehow, the idea of using it as the L/R in FFVII, which makes a character start the animation for trying to 'Run Away', brings ideas of hilarity to mind.
Like sitting on the couch with eyes glazed over pressing buttons until OMFG I CANT FUCKING FIGHT THAT!@1!!!! and frantically running circles around your living room hoping you get away.
