@djricekcn: I think the issue is that open world games take an insane amount of time to QA. With all of the possibilities that occur. Since its not like standard games that put players along a set path which makes testing a lot easier.
Same goes for other crazy shit like the Romance of the Three Kingdom games is another example of a huge fucking nightmare when it comes to QA.
Though with the DLC issues though not quite too sure what was up with that since those should have been caught right away.
Also anything that has save game issues should have been failed 100% by Sony during the approval process for the PS3 version due to the TRC rules.
The problem is that the Canadian (bilingual) release has a different product code than the USA one. The DLC released on PSN does not work with the Canadian release either, it states it only works with BLUS 30185, which is the USA release.
I suspect the problem with the Game of the Year Edition is they just used the same version for both this time, so people with Canadian Fallout 3 saves can't use them.
It should be able to be rectified with a patch, you would think.

I won't play EA games online. Period. Having their own separate servers is even more annoying than the "it's in the game" slogan (where the hell else would it be?). Therefore, I don't play EA games online. Ever. Haven't tried Rock Band 2 online tour yet, but if it wants me to sign up for EA servers, I'm gonna vomit.
